You can file your Social Security disability application online at www.SSA.gov or by calling 1-800-772-1213 to schedule an appointment with a Social Security representative.

What to Gather Before You File
Before submitting your application, make sure you have the following: 

  • Your Social Security number
  • Dates of any marriages
  • Your spouse’s date of birth and Social Security number (if applicable)
  • Your children’s ages

Social Security will ask about your employment for the past five years, including:

  • Names of employers
  • Job titles and duties
  • Dates you worked
  • Your earnings

The most critical part of your application is your medical history. Be thorough and include:

  • All of your medical conditions (both physical and mental) – don’t just list the most serious ones. Social Security considers all impairments together when determining disability.
  • Names, addresses, and phone numbers of all your doctors, specialists, therapists, and healthcare providers.
  • A list of current medications and conditions they treat.
